and Licensed Practical Nurses (LPNs)
work with your doctor to provide you skilled care
services at home. They provide high quality care
through accurate assessment, skilled treatment,
and cutting edge intervention with unconditional
support and compassion. A registered nurse is
available for admissions, urgent situations or
questions 24 hours a day, 7 days a week. Our certified
Nursing Assistants assist with activities such
as bathing, dressing, and meal preparation. Attention
to personal needs is a vital component of our
services.
Physical
Therapy
Our physical therapy staff will come to your home
to provide therapy such as therapeutic exercise,
gait training and instruction in using a walker
or cane and safety measures. They work with you
to return you to your pre-illness physical condition.
Occupational
Therapy
Occupational Therapists help patients regain day-to-day
skills such as eating, dressing, housekeeping
and personal hygiene, helping with maintaining
and improving your fine motor skills.
Speech
Therapy
Speech therapy can help patients improve speaking
or swallowing problems. We help patients that
have suffered from a stroke rehabilitate to their
maximum speech potential.
Medical
Social Services
Our Medical Social Services staff helps patients
and family members understand and cope with problems
caused by illness or injury. We connect our patients
to community resources and support them to living
and functioning at their maximum potential.
Equipment
and Supplies
Our nurses and therapists can provide names of
medical equipment companies or connect with them
on your behalf to supply any necessary medical
equipment for our patients to regain their independence
or become as independent as possible.
24 Hour
Registered Nurse on call Availability
Patients of Health1st Home Health Services have
access to a Registered Nurse (RN) 24 hours a day,
7 days a week. All you have to do is pick up the
phone and dial direct to the on call nurse. A
phone number is provided to you upon admission.
There is no added cost for this service.

Roles and Responsibilities
of Health1st Home Health Services Clinical Staff:
- Help patients and families acquire the knowledge
and learn the skills to manage their illness
and disease at home.
- Collaborate with the physician to maximize the
patient's return to pre-illness health status.
- Tailor home health services to the unique needs
of the patient/family:
- Assisting with medication management.
- Promoting recovery following
surgery, e.g., wound care, exercise and mobility
training, and adapting to the home environment.
- Managing the symptoms of chronic
conditions such as congestive heart disease,
chronic lung disease, and/or rehabilitation
following a stroke.
- Coordinating with other community agencies for
services the patient or family may need.
