Continuity of Care is
Key
At Health1st Home Health Services we understand
the important role continuity plays in your overall
care and rehabilitation. For your convenience,
we service a nine county radius in the greater
Philadelphia area. To ensure that
you receive uninterrupted health care services,
our home health staff is available upon physician
request in order to evaluate your need for home
care prior to your discharge from the hospital.
If this in-depth assessment indicates that home
care would be appropriate and beneficial to you,
a home health staff member may personally meet
with you and your family. This face-to-face meeting
not only eases the transition between hospital
and home, but also allows our home health staff
to get to know you and your family. After your
in-home patient evaluation, a home health nurse
will work in partnership with your physician to
develop a customized care plan to meet your individual
needs.
At Home with a Team Approach
Our home health team consists of health professionals
with specialized training in cardiopulmonary care,
diabetic care, geriatric care, mental health,
cancer care, orthopedic care, wound and skin care,
maternal-child care, neonatal care, HIV/AIDS care
and bone marrow transplant care.
Our multi-disciplinary team includes:
- Registered Nurses, including infusion and wound
care specialists
- Physical Therapists
- Occupational Therapists
- Social Workers
- Home Health Aides
Together, our multidisciplinary
team can work to provide you with individualized
expert medical care in the comfort of your own
home. Throughout the course of treatment, our
home health team will work in partnership with
your physician and provide him/her with ongoing
updates concerning your progress. If there are
any significant changes in your health status,
your personal physician will be contacted immediately
to advise us on alternative treatment options.

Our home health team is dedicated to providing
quality patient care with a personal touch. During
our home visits, we not only perform prescribed
treatments, but we also closely monitor any changes
in your condition. Just as importantly, we take
the time to listen compassionately and respond
to any questions, concerns, or special needs that
you or your family may have. In addition, we involve
family members in care planning and delivery,
as well as provide them with the resources, education
and/or emotional support they may need to better
care for you during your recovery.
